Santyl (collagenase) is an enzymatic ointment essential in modern wound care, specifically designed to break down dead necrotic tissue and slough without harming healthy, pink granulation tissue. While highly effective for pressure, vascular, and post-surgical wounds, its high cost—up to $300 per tube—makes proper application critical.
To ensure optimal performance, Santyl must be applied daily to a moist, saline-rich environment; dry dressings or silver-based products (with rare exceptions like Aquacel Ag) will deactivate the enzyme. For tough, black eschar, the wound must be surgically debrided or scored first to allow penetration. Ideal compatible dressings include hypertonic gauze like Mesalt or specialized foam dressings like Hydrofera Blue and Allevyn. Once the slough clears and healthy, beefy granulation tissue forms, Santyl treatment should stop immediately to avoid wasting product. Apply a nickel-sized amount directly onto the dressing daily for best results.
